We currently accept:

Blue Cross Blue Shield PPO plans

Insurance participation may change, and patients are encouraged to verify their benefits prior to requesting an appointment.

Private Pay/ Out-of-Network Fee Structure

We offer self-pay services only for:

Uninsured patients
Patients with out-of-network insurance plans

Please contact our office for current self-pay fees.

Important Billing Information

Patients are responsible for understanding their insurance coverage, co-pays, deductibles, and co-insurance.

We do not submit claims for out-of-network benefits on behalf of patients.
Self-pay is not available for in-network plans.

Self-pay fees are due at the time of service.
